What to Bring to Your Appointment
Please bring all necessary items to your appointment:
- Photo ID
- Insurance card
- A list of current medications
- Any previous imaging scans such as X-rays (if available)
Where Are We Located Inside Farmington Health Center?
Our podiatry clinic is located on the second floor on the northwest side of the building. When you arrive at our clinic, our staff will help guide you where to go.
We also offer free wi-fi inside our clinic and comfy furniture with electrical outlets so you can charge your devices while you wait.
Front Desk Check-In
You should arrive 15 minutes early to complete any necessary paperwork. This helps us dedicate as much time as possible to your visit.
During Your Visit
Your appointment will take place in an exam room. Your provider will talk to you about your medical history and symptoms. Tell your provider about your questions or concerns.
Your provider will also perform a physical exam to evaluate your condition. They will go over any medical imaging you might have.
Your specialist will provide information about your diagnosis and answer any questions. You will discuss available treatment options.
Next Steps
At the end of the appointment, you will have a care plan in place. Your provider may recommend tests or imaging studies. They may also refer you to another specialist such as a physical therapist.
